About this calculator

An antral follicle count (AFC) is an ultrasound count of the small resting follicles (2–10mm) visible in each ovary at the start of a menstrual cycle, and it's one of the main markers doctors use to estimate a woman's remaining ovarian reserve. This calculator simply adds the left and right ovary counts to get total AFC and places it into a reserve category.

Roughly speaking, a total AFC of 15–30 is considered normal reserve, under 5–7 suggests diminished reserve, and above 25–30 can flag polycystic ovarian morphology — though exact cutoffs vary by lab and age. AFC is measured by transvaginal ultrasound, usually on cycle day 2–5.

Reproductive endocrinologists and fertility specialists use AFC alongside AMH (anti-Müllerian hormone) blood levels to predict how a patient will respond to ovarian stimulation in an IVF cycle — a higher count generally means more eggs retrieved.
