Due dates calculated from an IVF cycle use the embryo transfer date instead of the last menstrual period, because IVF pinpoints the exact age of the embryo at transfer — a day-3 cleavage-stage embryo or a day-5 blastocyst — which is far more precise than estimating ovulation timing from a reported LMP. The standard method is: due date = transfer date + 266 days − embryo age at transfer (so 263 days out for a day-3 transfer, or 261 days for a day-5 blastocyst transfer), which mirrors the standard 40-week (280-day) pregnancy count measured from a theoretical last period.

Fertility clinics and the patients going through IVF or FET (frozen embryo transfer) cycles use this transfer-based calculation because it removes the ambiguity of a patient's actual ovulation date, giving a more reliable estimated due date for prenatal care scheduling than a standard LMP-based estimate would. It's presented to patients right after a positive pregnancy test to set expectations for the pregnancy timeline ahead.
