About this calculator

The HOMA-IR (Homeostatic Model Assessment of Insulin Resistance) is a widely used surrogate marker for insulin resistance derived from a single fasting blood draw, avoiding the need for a full clamp study. It is calculated as HOMA-IR = (fasting glucose × fasting insulin) ÷ a constant — 405 when glucose is in mg/dL and insulin in µU/mL, or 22.5 when glucose is in mmol/L — with higher values indicating that more insulin is needed to maintain a given fasting glucose level, i.e. greater resistance.

This calculator takes fasting insulin and fasting glucose and returns the HOMA-IR value. Endocrinologists, primary care physicians, and clinical researchers use it to screen for insulin resistance in patients with obesity, PCOS, or a family history of type 2 diabetes, and to track how resistance changes in response to weight loss, medication, or lifestyle intervention over time.
