About this calculator

The Maddrey discriminant function (DF) is a validated clinical score for assessing the severity of alcoholic hepatitis, calculated as 4.6 × (patient's prothrombin time − control PT, in seconds) + serum bilirubin (mg/dL). It combines a measure of liver synthetic function (PT) with a measure of cholestasis and hepatocyte injury (bilirubin).

A DF of 32 or higher defines severe alcoholic hepatitis and is the threshold clinicians use to consider corticosteroid therapy, since patients above this cutoff face significantly higher short-term mortality without treatment. Hepatologists use it alongside newer scores like MELD to guide steroid decisions.
