About this calculator

The insulin-to-carb ratio (ICR) tells a person with diabetes how many grams of carbohydrate one unit of rapid-acting insulin will cover, and it is commonly estimated with the “500 rule”: divide 500 by the total daily insulin dose (TDD), including both basal and bolus insulin. A TDD of 50 units, for example, gives an estimated ratio of 1 unit per 10 grams of carbohydrate.

People with type 1 diabetes and their endocrinologists use this ratio as a starting point for carbohydrate counting at meals, adjusting the dose up or down based on real-world blood glucose response.
