Pediatric eGFR (Schwartz) Calculator
Estimate pediatric eGFR.
Result
How it is calculated
The tool applies the bedside Schwartz equation for estimated glomerular filtration rate in children: eGFR = 0.413 × height / serum creatinine. Height is in centimetres and serum creatinine in mg/dL. The result is in mL/min/1.73 m² and is rounded to a whole number. Creatinine must be greater than zero. The constant 0.413 belongs to this bedside version of the equation and assumes creatinine measured by an enzymatic or standardised method. The equation is meant for children and adolescents; it is not valid for adults or infants in the first weeks of life. Interpretation belongs to a clinician.
Worked example
A child is 110 cm tall with a serum creatinine of 0.6 mg/dL. Multiply height by the constant: 0.413 × 110 = 45.43. Divide by creatinine: 45.43 / 0.6 = 75.7. Rounded to a whole number, the estimated GFR is 76 mL/min/1.73m². The same child with a creatinine of 1.2 would have half that value, about 38.
Common mistakes
- Entering creatinine in µmol/L. The equation expects mg/dL; divide µmol/L by 88.4 first.
- Entering height in metres or inches instead of centimetres.
- Using the tool for an adult, where other equations are appropriate.
- Treating a single estimate as a diagnosis. Creatinine can vary with muscle mass, hydration and the lab method used.
About this calculator
This calculator estimates a child's glomerular filtration rate (eGFR) using the bedside Schwartz formula (eGFR = k × height / serum creatinine, with k = 0.413 in the updated 2009 version), a simplified, height-based equation designed specifically for pediatric patients rather than the adult formulas like MDRD or CKD-EPI.
Pediatric nurses, physicians, and nursing/medical students use it to quickly estimate kidney function in children from a height measurement and a creatinine lab value, since children's muscle mass — and therefore creatinine production — differs enormously by age and size compared to adults.
