About this calculator

The SYNTAX score (SYNergy between percutaneous coronary intervention with TAXus and cardiac surgery) is an angiographic scoring system cardiologists use to grade the anatomical complexity of coronary artery disease from a coronary angiogram. It weighs the number of significant lesions along with high-risk features of each one — bifurcation involvement, calcification, chronic total occlusions, and vessel tortuosity among them — into a single composite number.

The score is central to the decision between PCI (stenting) and CABG (bypass surgery): scores are generally grouped into low (≤22), intermediate (23–32), and high (≥33) complexity tiers, with higher tiers favoring surgical revascularization in major trials such as SYNTAX and FREEDOM. This calculator produces a simplified estimate from lesion count and key features for educational purposes — it does not replace a formal angiographic reading by a cardiologist.
