About this calculator

Modern vancomycin dosing guidelines (ASHP/IDSA/PIDS/SIDP 2020) moved away from trough-only monitoring toward AUC24-guided dosing, targeting an area-under-the-curve of roughly 400-600 mg·h/L over 24 hours — high enough for antibacterial efficacy against MRSA, low enough to limit nephrotoxicity risk.

Using a first-order pharmacokinetic clearance model, the daily dose can be estimated from the target AUC24 and the patient's creatinine clearance (which drives how fast vancomycin is eliminated).
