About this calculator
Where fat sits on the body matters more for metabolic health than total body weight — waist circumference and the waist-to-height ratio (waist ÷ height) both specifically flag visceral abdominal fat, the kind that surrounds internal organs and is more metabolically active and inflammatory than fat stored elsewhere. A waist-to-height ratio above 0.5 is the simple, widely cited threshold researchers use to flag elevated risk, since it works consistently across different heights in a way that a single waist-circumference cutoff can’t.
This matters clinically because two people with an identical BMI can carry very different metabolic risk depending on where their fat is distributed — an “apple-shaped” body (fat concentrated around the abdomen) carries meaningfully higher risk of type 2 diabetes, cardiovascular disease and metabolic syndrome than a “pear-shaped” body (fat concentrated in hips and thighs) at the same weight, which is exactly why the American Heart Association and International Diabetes Federation both include waist circumference as a metabolic syndrome criterion alongside blood pressure, triglycerides and blood sugar rather than relying on BMI alone.
